Forms

In addition to any “event specific” forms, your student needs to have a Oak Grove UMC Medical Release Form on file in order to attend an event. The Oak Grove UMC Medical Release Form is valid for one year from the date that you complete it. When we receive confirmation that your student has registered for an event, we will send you an email regarding the status of your student’s forms. Please email youth@ogumc.com with any questions regarding these forms.

You can turn the form in one of three ways:

1) Bring it to the church office

2) Have your student bring them to any OGS Volunteer prior to the event

3) Mail them to the church:
